Prior to a youth being accepted as a resident of Full Potential Youth Services, an application for admission must be completed and submitted, and supporting documentation must also accompany the resident’s application. The following information should be submitted for initial consideration of admission:
Ø A completed Application Form
Ø Social History completed within the past year
Ø Medical History to include a physical examination over the past six months
Ø Psychological Evaluation complete within the last 12 months
Ø School/Education Information to included report cards, transcript, and earned credits during the current or most recent school year
Ø Prior Placement History and Discharge Summaries from previous placements (if applicable)
Ø Court Reports (if applicable)
Ø Copies of most recent IEP

At time of admission, the following documents must accompany the new resident:
Ø Physical examination form (dated no more than 90 days before admission) including a complete record of immunizations and a tuberculosis test dated within the previous year
Ø Medicaid/Insurance Card
Ø Social Security Card
Ø Birth Verification or Certificate
Ø Most recent School Records
Ø Purchase of Service Order
The resident’s parent, legal guardian, or agency representative must sign a Placement Agreement provided by Full Potential Youth Services on the day of placement.
Full Potential Youth Services does not accept emergency admissions or self-admissions.
